通过将空间概念与健康传播的文化中心路径理论相结合，研究以河北省M村和X村的村民参与健康体检的鲜明差异为切入点，分析造成差异的背后原因并提出乡村健康传播的改进策略。家庭空间的割裂或弥合影响了拆分式家庭中乡村留守老人与子女沟通健康决策的成本和心理负担，初步导致差异形成；熟人空间中外人或自己人的区别决定了村民对村医的信任程度，影响着村民们对村医体检号召的响应情况，成为差异形成的扩大机制；对表征性空间和地方空间的选择或平衡成为差异形成的保障机制，有无强大的结构性依托和健康政策兜底则影响了村民们主体能动性的积极或消极实践。
